Remit
As part of the Angus Community Planning Partnership, the Community Care and Health Executive Group (CCHEG) focus on ensuring the implementation of agreed care group strategies for adults with community care needs. This executive group also monitors progress against agreed outcome measures and reviews the actions of the associated collaborative groups which have an impact on the effectiveness of the partnership.
Role
The role of the Community Care and Health Executive Group is to oversee the strategic development of the community care partnership and to ensure compliance with local and national policies and priorities.
Remit
- To agree specified outcomes and priorities for individual Care Groups
- To approve and sign off individual Care Group Strategies
- To monitor and review progress in demonstrating outcomes
- To review population needs, including unmet need based on current service delivery and define areas of priority across different care groups.
- To oversee the allocation of resources between different care groups.
- To ensure appropriate links are made with Tayside-wide strategic planning initiatives, commissioning and delivery processes.
- To ensure appropriate links are made with other thematic groups within the Community Planning Partnership
- To identify and monitor risk
- To ensure effective communication of decisions

Managing and Monitoring the Single Outcome Agreement (SOA) for 2009/12
The CCHEG will be responsible for the management and monitoring of the following outcomes contained within the Angus Community Planning Partnership's Single Outcome Agreement for 2009/12, specifically National Outcomes 6, 7, 11 and 15.
The partnership will ensure that appropriate indicators and targets are agreed to support the delivery of the outcomes. Progress reports will be produced every six months and presented to the ACPP in January and June of each year.
The partnership will develop a delivery plan against each of the outcomes with specific actions identified for each of the partner agencies. A highlight report on progress against these actions will be produced every six months and presented to the ACPP in January and June of each year.
The partnership will ensure that indicators, targets and actions "below the waterline" demonstrate clear accountability to individual agencies and/or departments. Officers of the partnership will ensure appropriate links are made to relevant strategies and/or plans. Linkages to the national outcomes will be clearly defined.
An analysis of the data should be undertaken at least once per year and inter-dependencies between outcomes of the other thematic groups will be considered in reports to the ACPP Implementation Group on a regular basis.
